Procedures for Requesting Disclosure/Correction/
Suspension of Retained Personal Data

1. Identification of Retained Personal Data Subject to Disclosure Requests

We ask that you identify the retained personal data subject to the disclosure request. Please note that the retained personal data applicable to the following is not subject to disclosure.

(1) Data not applicable to retained personal data

・Data which we do not have the right to disclose because our firm has undertaken a consignment

・Data scheduled to be deleted within six months

(2) Data not subject to disclosure according to the provisions in Article 25 of the Act on the Protection of Personal Information

・Data which there is a risk of damaging the rights or interests of you or a third party by disclosure

・Data which there is a risk of considerable impairment to the performance of appropriate business in our firm by disclosure

・Data which there is a risk that disclosure may violate other laws

(3) Documents that have already been given to you (e.g. lease certificates, return tickets and receipts)

(4) Other data stipulated in laws

2. Disclosure Request and Inquiry Desk

If you wish to make a request for disclosure, please enclose the necessary documents to confirm your identity on the prescribed request form and a self-addressed stamped envelope with the address of the person requesting disclosure written on it and a postage stamp worth 612 yen affixed before sending it by post to the address below.

*We may disclose some information in a method other than in writing at the reception places below with your consent according to your request. In this case, we will make the disclosure only if we can confirm your identity.

3. Documents (Forms) to Provide When Requesting Disclosure

(1) If you are making the request for yourself:

If you are making the request for yourself, please first download and print our prescribed request form (A). Next, please complete the prescribed matters on the form and sign/seal it. After that, enclose the documents (B) to confirm your identity and then send them to our English Service Desk in 2 above together with a self-addressed stamped envelope (C) onto which you have written the address of the person requesting disclosure and affixed a postage stamp worth 612 yen.

B: Documents to confirm your identity

Please enclose a copy of one of the below. Please copy the part that indicates your name and address. If the certificate describes your honsekichi (permanent domicile), please make a copy after hiding the part of your honsekichi such as by affixing paper to it.
Driver's license, passport, basic resident register card, koseki shohon (official copy of part of the family register; only your address and name is required), certificate of residence (only your name and address is required), various insurance cards (medical records not required), nursing care card, pension book or pension certificate

*If your current address and registered address differ due to a move, please provide a copy of your certificate of residence showing the history of your move.
*If we cannot confirm your identity because you have not enclosed the document confirming your identity or because there are places that are unclear on the document confirming your identity, we will contact you to that effect and then return the documents you have provided us. We apologize for the inconvenience, but please re-submit the documents.

C: Self-addressed stamped envelope with the address of the person requesting disclosure written on it and a postage stamp worth 612 yen affixed to it

Breakdown of the 612 yen: Standard mail of 82 yen + registered mail of 430 yen + receipt only by the individual in question mail of 100 yen

D: Disclosure request fee

There is no disclosure request fee.

(2) If you are making a request as a representative for another individual

If you are the statutory representative of the individual in question or a representative authorized by the individual in question, please enclose the documents below (E or F) together with the documents in the case of making a request for yourself in (1) above.

E: Documents proving your status as statutory representative (E-1 or E-2)

E-1: Documents proving your status as the statutory representative of a minor (under the age of 20)

・ Documents to confirm that you are the statutory representative

If you are exercising parental authority for the individual in question: Koseki shohon (official copy of part of the family register) of the individual in question or a document equivalent to the Japanese koseki shohon
If you are a guardian of a minor who is the individual in question: Koseki shohon (official copy of part of the family register) of the individual in question or a document equivalent to the Japanese koseki shohon

E-2: Documents proving your status as statutory representative of an adult ward

・ Registration certificate stipulated in Article 10 of the Act on Guardianship Registration

F: Documents proving your status as a representative by authorization

・ Power of attorney

*If we cannot complete the procedures due to insufficient or defective authority of representation confirmation documents (also including the documents confirming your identity as a representative), we will contact you to that effect and then return the documents you have provided us. We apologize for the inconvenience, but please re-submit the documents.
If the necessary matters on the request forms have not been completed, have been entered incorrectly or are unclear, or if the self-addressed stamped envelope is not sufficient, we will contact you to that effect using the information you have provided us on your form. However, please understand that we will treat it as though you have not made a request for disclosure if you do not re-submit the documents within one week from when we contact you (including when documents fail to arrive).
*Please understand that we will not return the documents you submit when requesting disclosure (including the documents confirming your identity).

4. Start of Disclosure Procedures

We will start the disclosure procedures when all the documents you submit to make a disclosure request reach the English Service Desk and have been received by our firm. Please understand that our firm can take absolutely no responsibility for any postal troubles while disclosure requests have been sent or for their failure to arrive except for reasons attributable to our firm.

5. Method of Responding to Disclosure Requests

We will respond in writing by registered mail limited to receipt by the individual in question with the self-addressed stamped envelope. We will need at least four weeks to provide a reply from the start of the disclosure proceedings. Moreover, if the desk is busy or if there are other business circumstances in our firm, please understand that our response may take even longer than that. If there is a postal trouble or non-delivery after we have sent your our reply according to the above, please understand that our firm cannot take any responsibility except for reasons attributable to our firm.

6. Purpose of Use of Personal Information Acquired with Disclosure Requests

We will handle the personal information on the request forms and documents confirming your identity when making a disclosure request only within the scope necessary to respond to the disclosure request (e.g. to contact you about the disclosure request or to confirm when doubts arise with you).
We will dispose the submitted documents confirming your identity after retaining them for one year once we have given you a responce to disclosure requests.
